Seagrove Partners is currently offering a $25 Amazon Gift card for Diabetes patients to take part in an online survey.

Purpose: We are looking to gather user insight and perspective concerning your experiences with current diabetes therapy and products on the U.S. market.

Format:

• Follow the link below to complete the screener
• Once we have gathered the sample group we will email a link to the Online Survey
• Online Survey should take 10 to 25 minutes and must be fully completed
• Once the survey collection period closes (about a week) we will email out the $25 Amazon Gift Cards.

We are not gathering any personal patient information, nor do we share personal information with outside parties. Survey results are strictly to glean insights in overall trends.
